About your Role :
As our Mental Health Professional, you will work with adults aged 18 and older who have been discharged to lesser levels of care such as an adult family home or Assisted living facility, and need additional behavior supports to maintain that placement. By engaging the client and their support system, you will work together to create a Safety and Crisis Plan and Individual Behavior Support plan, and meet weekly to review and work toward those identified goals. Through ongoing evaluations, progress notes, plans, and meeting documentation, you will document client progress and / or behaviors to address. This position works an after-hour phone crisis response approximately 1 week every 3 months.
