Description
Clarvida is looking for a Full-Time Nurse Practitioner to join our Catalyst program , under the direction of a medical doctor performs comprehensive psychiatric evaluations, assessments, diagnosis, medication management, evaluation and on-going treatment of clients with mental disorders.
Catalyst
is an Assertive Community Treatment (ACT) program. Catalyst provides full service partnership services to Transitional Age Youth (TAY) ages 16 to 25 with a serious mental illness, and who are homeless or at risk of becoming homeless in San Diego County. Catalyst’s ACT team provides comprehensive, individualized services in an integrated and continuous manner. Services are collaborative and recovery-oriented, and treatment includes : psychiatric assessment, medication management, individual therapy, substance abuse treatment, co-occurring disorders treatment, supportive housing, resource linkage, advocacy, family psycho-education and therapy, supportive employment and education development, legal assistance, community integration services, peer support and group counseling.
PAY : $153,920-160,000 annually
Essential Duties / Responsibilities
- Conducts initial client assessment to include obtaining psychiatric and medical history, performing psychiatric assessment and overall health assessment for any presenting problem
- Orders necessary laboratory studies, monitors and follows-up on abnormal results
- Provides prescriptions for psychiatric medications under the advice and direction of a medical doctor
- Assesses and evaluates responses to medications and monitors for potential side effects or adverse reactions
- Consults with other members of the treatment team on clients’ treatment plans
- Documents using Anasazi Electronic Health Record, a complete assessment and plan including all medication changes on each client and all other mandatory documents as designated by Program Director
- Obtains medication consents when appropriate
- Participates in staff in-service and case presentation meetings
- Consults with medical doctor regularly on treatment plans for clients
- Perform duties as designated by ACT model treatment modality
- Perform Home visits and off-sites visits as required by the needs of the families and participants
- Provide support and perform other duties as assigned or necessary
- May provide direct supervision to any LPT or LVN’s on staff
- Works under minimal supervision by Program Director and Staff Psychiatrist
