POSITION TITLE: Targeted Case Managers (TCM)
REPORTS TO: Director of Case Management
CLASSIFICATION: Non-exempt
POSITION SUMMARY
CLASS’ Targeted Case Management services assist individuals with intellectual or other developmental disabilities to achieve maximum independence, integration, productivity, and successful inclusion in the community. Targeted Case Management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the services that are necessary to effectively support the individual in their community.
QUALIFICATIONS:
Education: Bachelor's degree in psychology, education, social work, or related field.
Experience: A minimum of at least six months experience in the field of services to individuals with developmental disabilities or additional relevant experience in the field of services to individuals who are developmentally disabled which may substitute for any portion of a bachelor's degree at a rate of six months of experience for each semester.

2) Comply with all federal, state, and local laws regarding communicable and infectious diseases and
CLASS LTD policies rules and practices regarding medical examination or immunizations.
3) No records of convictions, administrative findings or history of abuse, neglect, or exploitation of
children or adults unable to protect their own interest, and conviction of other crimes, including, but
not limited to crimes against persons, sex crimes, crimes against property, violation of personal rights,
public safety, public morals, or any crimes that would be classified in Kansas as a felony or deemed unacceptable for employment with CLASS LTD.

6) Compose written material as required for case notes, individual plans, etc. frequent use of computer
required.
7) Adhere to all Targeted Case Management (TCM) training requirements
8) Work as an effective member of the team(s) to which assigned.

ESSENTIAL FUNCTIONS
1) Helps clients assess their wants and needs, and meet their preferred lifestyle, choices within and outside of CLASS (i.e., employment and living options, HUD, SSI, SS, SDSI, doctors, VR, and other service providers, etc.)
2) Assists clients, as needed, with arranging and facilitating person-centered planning meetings, and other meetings, which may be needed or desired.
3) Facilitate the PCSP process and the development of action plan to help clients achieve their goals.
4) Reviews all plans implemented to ensure accuracy of information and monitor the effectiveness of strategies and progress toward achievement of desired outcomes.
5) Coordinates annual HCBS functional assessment (BASIS) for clients in the month they are due by gathering all data and sending to the CDDO assessor by the required deadline and inviting all required parties to attend.
6) Maintain accurate information for the annual BASIS assessments.
7) Reviews clients PCSP (Person-Centered Support Plans) for currency and adequacy in providing adequate information for the continuity of services.
8) Orients clients to and assists them, as needed, with CLASS LTD’s, CDDO, or any other provider/community resource grievance procedures.
9) Ensures clients receive annual training of their rights and responsibilities.
10) Communicates effectively and works in collaboration and cooperation with clients, families/guardians, MCO Care Coordinators, CDDO, community partners and team members.
11) Communicates significant information to Director of Case Management (DCM) and works with DCM to determine the appropriate response and which care team members, who have a need to know.
12) Research options and helps access assistive technology and other viable resources, that will enhance the ability of persons to function more independently in their homes, jobs, and the community.
13) Assists with developing and monitoring living arrangements that meet the lifestyle choices.
14) Ensures client records are complete and compliant with Class P&P as well as KDAD’s licensure requirements.
15) Works cooperatively with and facilitates teams centered around clients for assessment, planning, accessing needed resources, and monitoring quality of services.
16) Exhibits ability to be flexible in work schedule and has strong time-management skills.
17) Adheres to the TCM training requirement set forth by KDADS and CLASS LTD.
18) Provides accurate information to the Director of Case-Management (DCM) for outcome measures.
19) Shares resource knowledge of new programs, changes in funding or service deliveries, and community events that impact the lives of clients with all necessary team members.
20) Provides information, completes reports and attends meetings as needed, for the Human Rights Behavior Management Committee.
21) Use laptop and other office equipment provided by Class LTD to maintain accurate documentation used for client file/plan maintence as well as billing the services provided.
22) Meets all deadline requirements for billings and paperwork, accurately to ensure reliable documentation for individuals’ records.
23) Adheres to the rules, regulations and standards required for case management by KDADS.
24) Provides case management for persons on the waiting list when these services are requested.
25) Works cooperatively with CDDO and case-records personnel in the documentation and tracking of status changes and case-records information for clients using various platforms such as computers files and databases, to include Therap.
26) Reports abuse, neglect, exploitation and works cooperatively with CDDO, KDADS, and DCF Investigators during investigations.
27) Reports any incident requiring a GER/AIR to DCM, within 24 business hours of their being notified of the incident. If TCM completes GER/AIR regarding an outside TCM only client; TCM will notify DCM it was completed and send copy to DCM within 24 business hours.

29) Role models appropriate hygiene, appearance, etiquette, and behavior to clients.
30) Attends and participates in the Case Management Team Meeting, County Steering Team (for the counties they have clients), CDDO affiliate meeting, and any other assigned teams/meetings.
31) Maintains neat and orderly work area.
32) Coordinates with the CDDO & MCO Care Coordinator on pre-placement and intake processes for persons applying for services.
33) Completes monthly quality assurance checklists except those who are waitlisted. Communicates quality issues to service staff and submits report to CDDO by due date.
34) Maintains monthly contact with all family and/or guardians unless otherwise specified in the person’s PCSP.
35) Ensures that appropriate documents are completed for all clients (consents, service agreements, etc.).
36) Completes & submits status changes (as required) within 3 business days.
37) Attends IEPs, transition planning, and other meetings as requested by parent/guardian.
38) Facilitates development of IJP’s for clients who are involved with the criminal justice system.
39) Researches, locates, and advocates for natural supports, alternative funding sources and alternative services to meet client needs.
40) Reviews documentation necessary to assess client needs are met and follow up with services staff ensure corrective actions are taken, when needed.
41) Upon the death of client, notifies the Director of Case-Management (DCM) and will complete required discharge and death reports to close client file and send to CDDO.
42) Completes CLASS & CDDO discharge summaries as needed.
43) Facilitates development of behavior plans and risk assessments for clients with health or safety risks. Ensures restrictive plans are approved according to Article 63.
44) Other duties as assigned.
