We are currently seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join our team on a PRN basis in Brevard County, covering Melbourne, Palm Bay, Titusville, Cocoa, Cocoa Beach, and Merritt Island.

Position Summary

As a PRN Licensed Practical Nurse (LPN), you will provide skilled nursing care to patients in their homes, ensuring high-quality care and comfort. You will collaborate with the interdisciplinary team to carry out physician-ordered treatments, monitor patient progress, and educate patients and families on proper care techniques.

Key Responsibilities

• Provide direct nursing care, including wound care, medication administration, catheter care, and injections.
• Assist in monitoring and documenting patient progress, reporting changes to the RN and physician.
• Follow established care plans and contribute to ongoing patient assessments.
• Educate patients and caregivers on medication management, disease processes, and home safety.
• Maintain accurate and timely documentation of care provided.
• Ensure compliance with state and federal home health regulations.
• Communicate effectively with the care team, patients, and families.
• Participate in on-call rotation as needed.

Qualifications

• Current LPN license in the state of Florida.
• Minimum of 1 year of nursing experience, preferably in home health or a related field.
• Strong clinical skills with the ability to work independently.
• Excellent communication and interpersonal skills.
• Reliable transportation and willingness to travel within Brevard County.
• CPR certification required.
