Position Title: Bilingual Housing Case Manager
40 hours/week with Benefits
Purpose
The Bilingual Housing Case Manager plays a pivotal role in supporting individuals and families by bridging language barriers and connecting them with housing and essential services. They uphold the values of a community-based, client-centered, trauma-responsive organization, fostering inclusivity and accessibility for Spanish-speaking clients.
They will conduct assessments to determine client needs and eligibility for services, providing advocacy, education, and support to people in need.
Reports to: Social Services Program Manager
ResponsibilitiesClient Advocacy
The Housing Case Manager is responsible for representing clients and their families, empowering them through dynamic support, and amplifying their voices throughout the community to develop clients’ resourcefulness, resilience, and self-sufficiency.
- Determine client needs through formal assessments, interviews, and screening processes.
- Determine which programs will be most beneficial; qualify and connect clients to appropriate resources.
- Provide high-quality, trauma-informed case management services.
- Meet regularly with clients to build trust and confidence and to provide guidance and education.
- Create dynamic and effective plans for client success.
- Explore complex situations with clients, supporting them with problem solving methods and tools to provide immediate and beneficial assistance.
Administration of Programs and Data Quality Management
- Ensure timely and accurate documentation and data quality within Homeless Management Information System (HMIS) and other organizational systems.
- Stay updated on new policies and program requirements, continuously enhancing knowledge of available resources and programs.
- Collaborate with Case Managers and other staff to facilitate client access to resources and information.
- Cultivate relationships with community partners, landlords, and service agencies to ensure consistent support and services for Spanish-speaking clients.
Qualifications
- Fluency in Spanish and English, enabling effective communication with bilingual clients and collaboration with diverse stakeholders.
- Working knowledge of social services.
- Proficiency in using computer technology for data management and communication.
- Demonstrated ability to work autonomously and collaboratively within a team, with a commitment to engagement and inclusivity.
- Valid driver’s license, reliable transportation, and willingness to travel for business purposes.
- Preferred Bachelor’s Degree in Social Sciences or related field, or equivalent human services work experience.
Working Conditions and Physical Effort
- The employee must be able to work well in an office environment. Due to the nature of the work, the employee may be required to deal with angry, confused, and/or frustrated clients.
- The employee must frequently stand, sit, lift, and/or move up to 10 pounds, and occasionally lift and/or move up to 35 pounds.
- Requires considerable walking (including climbing stairs).
