Job Summary:

This individual will ensure that the HIV Prevention Program operates in accordance with grant deliverables. The HIV Prevention Coordinator will collaborate with community-based organizations (CBOs) and community partners to increase HIV surveillance, testing and education, specifically targeting high risk populations. The position will manage a diverse range of support, operational and programmatic activities and will supervise program staff and workload.

Primary Duties & Responsibilities:

1. Plans and manages all HIV related epidemiology programs for the health District.

2. Manages all aspects of the District’s HIV surveillance and Prevention unit.

3. Manages all HIV grant deliverables including reporting functionality.

4. Strengthens PrEP services for the District internally or in collaboration with community-based organizations.

5. Evaluates current HIV Programs and plans for District needs based on analysis of epidemiologic data.

6. Manages HIV program budget, ensuring appropriate expenditures of program funds in timely manner.

7. Manages contracts and agreements enacted with community partners to carry out program deliverables.

8. Communicates regularly with supervisor, providing information about the current program activities and caseload.

9. Communicates concerns/ issues promptly to supervisor.

10. Develops and disseminates regular programmatic reports of Program activity and disseminates reports to supervisor.

11. Develops an annual evaluation of the program, including activities, successes, and developmental goals.

12. Contributes programmatic information to the development of District health reports.

13. Maintains all State reporting requirements.

14. Performs or directs timely field investigations when appropriate.

15. Supervises the Linkage Coordinator to ensure provision of case management and linkage services as required under HIV grant deliverables including collaboration with CBOs and Public Health partners to facilitate linkage to care for HIV patients.

16. Provides technical guidance and education regarding HIV to District and clinical STD staff, private health care providers and the community in accordance with established medical protocol and District guidelines.

17. Manages HIV Program staff to meet programmatic deliverables.

a. Creates and maintains a high-performance environment characterized by positive leadership and a strong team orientation.

b. Defines goals and/or required results at beginning of performance period and gains acceptance of ideas by creating a shared vision.

c. Evaluates employees at scheduled intervals; obtains and considers all relevant informational evaluations and supports staff by giving praise and constructive criticism.

d. Motivates staff to improve quantity and quality of work performed and provides training and development opportunities as appropriate.

e. Immediately confronts an employee in a professional manner whenever a problem (i.e. performance, work conduct, work habits, attendance) is observed or brought to their attention.

f. Follows through with written documentation of the counseling (verbal/written), memorandum of concerns or expectations, reprimand or adverse action, including signatures of both the supervisor and employee and date of action.

g. Moves toward resolution of problem, up to and including dismissal, as quickly as possible, if employee fails to correct problem.

h. Adheres to established policies on privacy and security for compliance with the Health Insurance Portability and Accountability Act (HIPPA) as applicable by Agency Policy.

18. Prepares for Public Health emergencies and respond as directed.

a. Complete a personal/family emergency plan to ensure that you and your family's needs are taken care of if you have to respond to an emergency.

b. Complete training as required, including independent study and classroom emergency preparedness training, drills and exercises, as directed.

c. In the event of a Public Health emergency, participate in the emergency response activities, as directed by the District Health Director or Director of the Department of Public Health.

19. Develops and coordinates special projects and activities as directed by the District management.

20. Works closely with other units of Clayton County Health District to coordinate services and activities and to meet program and/or organizational goals.

21. Performs other duties as assigned to meet program and/or organizational needs.

Minimum Qualifications:

Bachelor's degree from an accredited college or university AND Two years of related experience OR Associate’s degree from an accredited college or university AND Four years of experience in a related area OR Six Years of experience in a medical related field OR Licensure as a registered nurse AND Three years of nursing experience OR One year of experience at the lower level Communicable Disease Spec 3 (HCP082) or position equivalent.

Preferred Qualifications:

Preference will be given to applicants who in addition to meeting the Minimum Qualifications also possess a background in the following:

Experience working with HIV programs. Experience performing partner services and linkage to care for HIV positive clients. Experience collaborating with community based organizations providing HIV prevention activities. Experience supervising /managing staff. Experience managing program deliverables and reporting.

Pay: From $55,000.00 per year
