Description :
The MDS Care Plan Coordinator performs nursing assessments of residents and documentation review and analysis as needed for completion of resident’s MDS, CAAs and Care Plan in accordance with standards of nursing practices, regulations, and RAI Manual. May lead or direct licensed and non-professional facility staff in the assessment and completition of MDS, CAA and Care Plan process.
Requirements :
Nursing degree from an accredited institution
Valid state licensure as LPN or RN
Two years related experience and / or training
Experience as a MDS / Care Plan Coordinator preferred
Long term care experience preferred
