Key Responsibilities
Perform patient assessment, establish plan of care, and prepare discharge plans.
Evaluate effectiveness of treatment and modify as necessary.
Assess educational and cultural needs of the patient, family, caregiver, or significant other and communicate accordingly to promote maximum benefit of care.
Coordinates with interdisciplinary team on patient needs, dietary requirements, treatment modifications, and progress.
