As a Licensed Practical Nurse with ClearSky Rehabilitation Hospital of Elwood-St. Joseph, LLC, you will be responsible for providing high-quality patient care under the direction of a Registered Nurse. You will work closely with our interdisciplinary team to develop individualized plans of care that meet the unique needs of each patient.

Responsibilities:
* Provide skilled nursing care to patients as ordered by physicians and under the supervision of a Registered Nurse
* Implement orders for medications and treatments as directed by physicians and according to established protocols
* Participate in evaluation of patient responses to interventions and seek guidance when necessary
* Report on patient care rendered, problems concerning patient needs, and resolution to those needs to supervisors
* Identify potential risks and contribute to the development of interventions to mitigate them
* Assist in identifying learning needs and reinforcing self-care techniques with patients
Requirements:
* Two years' experience in an inpatient hospital setting, with inpatient rehabilitation experience preferred
* Current state license as a Licensed Practical Nurse required
* Current CPR certification required
* Knowledge of current nursing theory and practice, infection prevention standards, and clinical operations procedures
* Ability to maintain quality, safety, and infection prevention standards
Working Conditions:
This position requires standing, walking, bending, reaching, lifting, pushing, and pulling for extended periods. Possible exposure to bodily fluids. Visual acuity required for patient assessment and documentation of care.
