Assessment Counselor
Bachelor's degree (LSW or RN preferred) or Master's degree in Social Work or Counseling with licensure.
Shift :
12-hour shifts
Full Time, Part Time, and PRN (Full Time : 3 12-hour shifts, with every other weekend rotation)
Days and Nights
Hours : 7a-7p or 7p-7a
The Role Itself
Coordinates care with interdisciplinary teams and external providers and updates treatment team on clinical information.
Participates in treatment planning, including psychosocial assessments.
Provides resource information and "warm-referrals" to community providers for engagement.
Completes thorough, timely documentation in compliance with standards.
Demonstrates crisis intervention skills and upholds confidentiality.
Completes the admission process, including verification of benefits and necessary authorizations.
