- Assist the transdisciplinary Early Intervention team in evaluating infants/toddlers to determine service delivery and plan outcomes based upon the needs of the child and family.
- Work with the Early Intervention team to complete a functional child and family assessment.
- Work with the IFSP team to develop an appropriate IFSP, which includes functional outcomes based upon the needs of the child and family and an appropriate frequency, duration, and intensity of services to meet the outcome(s).
- Serve as a Primary Service Provider to families of eligible children, to implement programming based upon the IFSP. May serve as a secondary service provider to support the assigned Primary Service Provider in implementing the IFSP.
- Coach families by using evidence-based early intervention practices and maintain confidentiality of the families served.
- Complete progress reports and maintain case notes and other documentation as required.
- Participate in weekly transdisciplinary team meetings and recommend children for teaming; provide quarterly updates as necessary.
- Assists in the transition of children to preschool by providing information as needed to the child’s EI team and/or IEP team/school district.
- Schedules home visits according to the needs and activities and routines of the family as much as possible.
- Delivers services in a natural environment which includes, but is not limited to, homes, community locations, outside locations such as parks or pools, etc.
- Participate in continued education to meet licensure requirements and to maintain knowledge of evidence-based practice.
- QUALIFICATIONS: Licensed Physical Therapist in the state of Ohio. Ability to lift, carry, and position children in a safe manner and perform crisis interventions. Successfully pass a pre-employment post-offer physical exam, criminal background check, and registry checks as required by Statute. Must meet physical demands of position as stated in job description, with or without reasonable accommodations EOE
