The role of the Breast Oncology Navigator will be a joint role to streamline care between the Women's Imaging Center and Breast Surgical Services. The RN in this role will be the primary caregiver responsible for coordinating care between the two service lines in order to minimize barriers between the diagnosis of breast cancer and subsequent intervention.
A caregiver in this position works from 8:00AM – 5:00PM.
A caregiver who excels in this role will:
Coordinate multidisciplinary clinic programs, program development and services.
Navigate the care continuum for patients by collaborating with the Medical Director and act as a "compass" for patients diagnosed and/or receiving therapy.
Ensure timely scheduling of diagnostic work-up, results reporting and communications between patients and the multidisciplinary team.
Promote the multidisciplinary team process and ensure patient receives necessary referrals and follow-up.
Provide efficient and effective care for patients and families and keep current with emerging technologies and treatment trends.
Coordinate cancer related patient/staff education programs, projects, services and groups.
Assist with community events, evaluate the results and continue outcomes research.
Compile and analyze Quality Improvement data and benchmarking.
Complete mandatory education and training to maintain organization and department specific competencies and requirements and certification/licensure.
Minimum qualifications for the ideal future caregiver include:
Current licensure as a Registered Nurse (RN) in state of employment
Basic Life Support (BLS) Certification
Any registered nurse or advanced practice nurse must obtain a cancer-specific certification or demonstrate ongoing qualifying education within the timeframe of the facilities accreditation cycle
Knowledge of oncology prevention diagnosis disease process and symptom management
Program completion in Professional Nursing
Five years of nursing experience including two years in oncology
Preferred qualifications for the ideal future caregiver include:
Bachelor’s of Science in Nursing (BSN)
Experience in case management mental health program development and community health/home care
Oncology Certified Nurse (OCN)
