Social Workers in this facility play an integral role in our mission to provide patients with comprehensive and holistic care. In this position, you will work collaboratively with the Case Management team to assess and plan interventions for families coping with various social, emotional, financial and environmental challenges. A caregiver in this position works days from 8:00AM – 4:30PM, with occasional weekend and one holiday requirements.
A caregiver who excels in this role will:
- Provide psychosocial assessment, counseling, crisis intervention and community resource assistance to patients and families utilizing a variety of treatment modalities, including individual, family, group and specialized counseling.
- Conduct psychosocial assessment interviews with patients and families to identify problems requiring social work interventions and treatment plans for appropriate services.
- Provide counseling to impact problems related to environmental needs, family or interpersonal conflict, grief issues, financial concerns, and coping with physical and/or mental illness using a variety of treatment modalities, including individual, family, group or specialized counseling.
- Intervene in crisis situations.
- Assist patients and their families in adjusting to episodes of acute and chronic illness, coping with lifestyle changes precipitated by illness or injury, and addressing issues of terminality.
- Provide consultation to Care Managers and other health care professionals when coordination with significant or intensive community resources is necessary to achieve desired treatment outcomes.
- Receive referrals for complex problem resolution from Care Managers, physicians and/or care team.
- Identify community resources and provide information and referrals to same.
- Update and maintain professional knowledge, resource information, skills and competencies related to areas of practice and assigned caseload duties by participation in department staff development and other professional education programs and by development of annual professional goals and activities to meet these goals.
- Document social work assessments, interventions, and psychosocial treatment plans according to department and medical center guidelines.
- Compile statistical reports and prepare specialty documents and correspondence.
- Participate in health system and departmental Performance Improvement Initiatives to ensure positive patient and family outcomes.
- Adhere to NASW Code of Ethics.
Minimum qualifications for the ideal future caregiver include:
- Master’s of Science in Social Work (MSW) with a casework concentration from a CSWE accredited program
- Ohio license appropriate to assigned caseload and practice area
- One to two years of direct practice experience
- Strong interview assessment, organizational, interpersonal, collaboration and problem-solving skills
