What are the responsibilities and job description for the Pharmacy Clinical Specialist - PediatricHematology/Oncology position at Cleveland Clinic?
Coordinates and provides thepharmaceutical care of patients for an assigned clinical unit andparticipates in patient care rounds with medical staffpersonnel.
Responsibilities :
Performspharmacotherapeutic monitoring and provision of pharmaceutical carefor patients in the area of service responsibility in a routine,continuous manner, taking into account the age and clinicalcondition of each patient.
Provides general andpatient specific drug information to patients, physicians, nursesand, and other health careprofessionals.
Interacts routinely withphysicians, nurses, and other health care professionals to provideoptimal pharmaceutical care topatients.
Supports clinical pharmacist andtechnician educational development by participating in departmentalstaff development programs and provides leadership in the educationof staff involved in providing pharmaceutical care topatients.
Supports clinical pharmacists inproviding pharmaceutical care to patients by direct interaction,and formal educational programs.
Supports andparticipates in pharmaceutical related research in area ofspecialty.
Maintains expertise in areas ofpractice through continual learning, journal review and othereducational activities.
Educates clinicalpharmacists, technicians, pharmacy students, post graduatetrainees, allied health professionals, nurses and physicians inpharmacotherapy within in the specialty area.

Education :
PharmD or Bachelors ofScience degree in Pharmacy from an accredited college of pharmacyand completion of a post-graduate residency or fellowship trainingrequired.
If hired for a specialty area wherespecialty residency training is available, then specialty residencyisrequired.
Certifications :
Current State licensure asa Registered Pharmacist (RPh) or eligible for reciprocation incurrent State within 90 days of hire. Board Certificationpreferred.
For individuals requiredto administer immunizations, current Basic Life Support(BLS) certification through American Heart Association (AHA)and completion of a Board of Pharmacy approved course in theadministration of immunizations or certification obtained within 90days of hire are required.
For individuals required to respond to adult orpediatric cardiopulmonary arrest events,current Advanced Cardiovascular Life Support (ACLS) orPediatric Advanced Life Support (PALS) Certification throughAmerican Heart Association (AHA), or certification obtained withinone (1) year of hire, isrequired.
Complexityof Work :

Must be able to work in astressful environment.

WorkExperience :
Completion of apost-graduate year 2 residency (PGY2) or fellowship requiredor
Completion of a post-graduate year 1residency (PGY-1) plus minimum of 2 years of experience as aclinical specialist or equivalent role which includesproviding pharmaceutical care and performing pharmacotherapeuticmonitoring including 50% or more of time spent in the specialtyarea or
Minimum of 4 years of experience as aclinical specialist or equivalent role which includes providingpharmaceutical care and performing pharmacotherapeutic monitoringincluding 50% or more of time spent in the specialtyarea.
PhysicalRequirements :
Typicalphysical demands include ability to perform the job while standingand walking for prolonged times, approximately 75% of shift andcontinuously for extended periods oftime.
Manual dexterity sufficient toperform computer and dispensing functions of the job; ability tolift up to 30 lbs.
Position often includesreaching, stretching, stooping, climbing steps andsitting.
Must have normal or correctedvision.
Must have ability to clearlycommunicate verbally by phone or inperson
