A caregiver in this position works evenings from 11:00AM – 11:30PM.
A caregiver who excels in this role will:
- Provide supervision, assessment, planning and direct patient care to hospice patients.
- Perform accurate and complete admissions assessments and re-assessments.
- Assess patients for safety risks, abuse and neglect.
- Assess, document and manage patient’s pain.
- Prioritize and document patient problems.
- Administer medications and treatments safely and accurately with a focus on patient comfort.
- Coordinate care for managing a caseload of patients.
- Document changes in patient condition and effectiveness and timeliness of care/interventions.
- Evaluate, update and revise plans of care.
- Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges.
- Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
Minimum qualifications for the ideal future caregiver include:
- Graduate from an accredited school of Nursing
- Current licensure as a Registered Nurse (RN) in the State of Ohio
- Basic Life Support (BLS) certification from the American Heart Association (AHA).
- One year of current clinical experience as an RN
- One year of experience in an acute medical-surgical or critical care setting
- Valid Driver’s License (VDL) and proof of automobile insurance with $100,000/$300,000 coverage required
Preferred qualifications for the ideal future caregiver include:
- Bachelor’s of Science in Nursing (BSN)
- Hospice and Palliative Care Nurse certification

Physical Requirements:
- Full range of body motion for handling and lifting packages, manual and finger dexterity and eye-hand coordination
- Standing and walking for extensive periods of time
- Heavy work, exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently and/or 10 to 20 pounds of force constantly to move objects
- Physical Demand requirements in excess of those for Medium Work
