Performs skilled technical work in Emergency Medical Services, functioning independently or as part of a healthcare team to provide assessment and care to individuals with chronic medical, behavioral, and social challenges and other duties as apparent or assigned. This position involves identifying patients' needs and facilitating access to available community and medical resources. The role emphasizes extending health services to underserved populations, supporting primary care, public health, disease management, prevention, and wellness efforts, as well as PORT program functions. Additionally, it includes providing 911 Quick Response Vehicle support and staffing Advanced Life Support (ALS) ambulances during emergencies, critical staffing shortages, or adverse weather conditions. Work is performed under the supervision of the EMS Director, with a typical schedule of Monday through Friday, 8:00 AM to 5:00 PM, though occasional nights and weekends may be required to ensure program and community needs are met, as well as being subject to 24/7/365 emergency callback excluding scheduled leave.
ESSENTIAL FUNCTIONS:
Performs paramedic-level care, including receiving referrals and scheduling visits to assess, treat, and coordinate health services for patients.
Conducts post-hospital follow-up care, including medication monitoring, dressing changes, vital sign checks, and coordinating follow-up with healthcare providers.
Provides patient education on topics such as diabetes management, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), injury prevention, geriatric assessments, prenatal care, and nutrition.
Maintains accurate documentation, including patient care records, incident reports, and supply inventory for the Community Paramedic Program.
Attends meetings, participates in community events, and provides training to personnel as needed.
Operates and conducts routine checks of emergency vehicles and equipment.
Assists with 911 response during emergencies or staffing shortages.
May assist in teaching EMT courses or other community education programs, as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
Thorough knowledge of EMS policies, procedures, and best practices. General knowledge of emergency care principles, including patient assessment, treatment, and transportation. Strong understanding of anatomy, physiology, and common medical conditions. Familiarity with local community resources and ability to connect patients to needed services. Ability to communicate effectively with patients, families,
healthcare professionals, and the public. Proficiency in maintaining accurate records and documentation. Capability to operate EMS equipment and vehicles safely and efficiently. Strong interpersonal skills and the ability to build trust with patients and community members.
MINIMUM EXPERIENCE & EDUCATION:
High school diploma or GED required and a minimum of two (2) years of experience as a Paramedic.
Associate's degree in EMS or a related field preferred but not required. Experience working with community organizations or in public health is desirable but not required.
PHYSICAL REQUIREMENTS:
Regularly requires exertion of up to 25 pounds, occasionally over 100 pounds. Frequent standing, walking, speaking, and hearing; occasional stooping, kneeling, and crawling. Work involves exposure to bloodborne pathogens, outdoor weather conditions, and wearing specialized personal protective equipment as necessary. Visual and auditory acuity is required for patient assessment and communication.
SPECIAL REQUIREMENTS:
Current North Carolina Paramedic Certification.
Current BLS HCP Provider/Instructor Certification.
Current ACLS and PALS Certifications.
Valid Driver's License.
This is position is subject to randomized drug testing.
