Certification Details BLS License / Certificate Verification MMR Hepatitis B Varicella Tdap 2 Step PPD OR IGRA (Quantiferon / Tspot) Job Details Rotating day / night 3x12 OR 4x12 hour shifts surgical oncology Some step down patients Ratios vary from 1 : 4-1 : 5 days and 1 : 6 on nights Job Requirements BLS - Required for clinical positions License / Certificate Verification - Primary Source Verification of professional license and / or certification, as required for the position ran within 30 days of start updated upon expiration. MMR - Positive MMR titers ONLY from within the last 15 years. Hepatitis B - Proof of full Hepatitis B series or signed declination. Varicella - Positive Varicella titers ONLY from within the last 15 years. Tdap - Proof of vaccination within past 10 years. 2 Step PPD OR IGRA (Quantiferon / Tspot) - 2-Step TB test with negative reads within the last 6 months. Schedule Information Rotating day / night 3x12 OR 4x12 hour shifts Unit Specific Information Some step down patients Ratios vary from 1 : 4-1 : 5 days and 1 : 6 on nights Additional Details Proof of vaccine series or completion and approval of client medical or religious exemption form. Preferred, not required. Physical Exam completed within one year of initial start date. Needs to be a Pre-Employment exam statement signed and must state that traveler is able to work without restrictions.
