Primary Source Verification of professional license and / or certification, as required for the position ran within 30 days of start updated upon expiration. Attestations not allowed. Needs to be rerun every 2 years with RN license renewal & upon expiration. Temporary Licenses are NOT accepted – must have a valid permanent license.
Fit Test : Completed within 1 year of start updated annually.
UHS Skills Checklist : Skill specific required to be complete within 1 year prior to start.
UHS Competency Exam : Must score at least 80%. To be complete within 1 year prior to start.
UHS Core Competencies : 80% passing score required, Completed within 1 year prior to start.
UHS Submission Packet : Resume to include Education and Work History, 2 Professional References required within the last 2 years, and Job Application to be completed by the candidate.
Physical Exam completed within one year of initial start date. Needs to be a Pre-Employment exam statement signed and must state that traveler is able to work without restrictions.
