Purpose of this position
The purpose of this position is to assess residents' physical and mental function and document data on minimum data set forms completely and accurately; document all additional assessments required completely and accurately; and determine appropriate referrals to other health care professionals; and to use the resident assessment protocols to determine whether to proceed or not proceed. This position will be responsible for the overall clinical oversight of the residents in his/her assigned unit. To Identify a key person on the infection prevention team having responsibility for the infections prevention program, dissemination of infection prevention information including surveillance data and policy decisions.
Job Overview
Interview residents, residents' representatives and others with information related to the minimum data set assessment.
Collect all data necessary to complete minimum data set forms and related documentation.
Make appropriate referrals to other health care professionals.
Participate and/or conduct interdisciplinary meetings.
Supervise and participate in the overall care of the residents
Oversee clinical coordination, review labs, assist the physician with rounds
Participate in the facilities Quality Assurance process to review facility systems as directed
Effectively direct, manager, and operationalize the infection prevention program
Requirements for this position
Registered Nurse RN or License Practical Nurse LPN
Great customer service
Strong interpersonal and time management skills
