- Conduct follow-up visits for home health patients to assess eligibility for continued services.
- Provide maintenance therapy and assist patients in reaching their rehabilitation objectives.
- Regular travel to patient residences is required, necessitating a valid driver’s license.
Qualifications :
Home Health experience preferred but not required
Valid California Physical Therapist Assistant license required
Current CPR certification required
Current California driver’s license, good driving record, and reliable transportation
