Pay Range: $34.00/Hr to 40.00/Hr based on experience

The Clinic RN is the administrative liaison for the assigned Primary Care Department. The Clinic RN is responsible for assisting the Nurse Manager with unit organization, day-to-day operations, and the clinical management of patients and staff personnel alike. The Clinic RN will collaborate with the Nurse Manager to establish and communicate goals with all staff, as well as monitor and measure patient care and safety outcomes. The Clinic Registered Nurse also serves at the registered nurse of the primary care clinic and is responsible for the care coordination of patients and working cooperatively with medical providers and other team personnel.
Essential Functions (Not all inclusive):
- Assesses and triage patient condition(s) via phone call or walk-ins. Establish level of care and facilitate treatment plan. Establish realistic and appropriate goals based on patient specific needs. Deploy appropriate Nursing actions to meet established goals, through utilizing appropriate resources. Manage the physical, psychological, social and rehabilitative needs of the patient, including discharge planning.
- Assists in the care coordination of patient (transitions of care, DME supplies, etc).
- Administers medications and performs procedures, as ordered by the medical provider.
- Room patients assuring appropriate and accurate information is input to the EMR.
- Administers vaccines as ordered by provider, verifying VFC vs. private status.
- Serves as the backup Vaccine Coordinator for the NYSIIS Program.
- Uses EMR communication noting that it is a legal entry.
- Completes vision, hearing, EKG, and POC testing as indicated.
- Patient education.
- Contact patients with Provider recommendations after laboratory results are reviewed
- Assist with implementation of quality incentive measures.
- Assist with resolution of problems/issues with patient/family/staff relations.
- Be familiar with and able to interpret and enforce all policies and regulations.
- Utilize and apply the American Nurses Association standards of nursing practice as a framework for professional responsibilities.
- Develop and foster positive staff morale and teamwork and be a positive role model.
- Serve as the backup Vaccine Coordinator for the NYSIIS program.
- Assist with day-to-day staffing.
- Supervises the daily activities of the department to ensure optimum performance.
- Works cooperatively with the Clinic Manager to develop and maintain systems for collecting information to measure department productivity.
- Collaborates with Patient Care team in obtaining information/planning on new
products/technology; facilitates the introduction of new equipment/supplies.
- Conduct department activity in the absence of the Nurse Manager.
Education, Training, and Experience:
- Associates Degree Require
- Current New York State License and Registration as a Registered Professional Nurse.
- Current BLS Certification
- 3-5 years of healthcare or related experience. Strong analytical, organizational, interpersonal and demonstrated customer services skills.
