Compensation: Approximately $22.00 - $27.00 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity.
Overview of Role:
Functions as a liaison and advocate for patients between their care team, specialty care, and/or other relevant staff and assists patients in navigating referrals to community resources and specialty care.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Completes comprehensive assessments to identify needs and barriers to the member’s ability to manage their medical and behavioral health conditions and treatment plans.
- Participates with the patient, caregivers, the primary care medical provider and health care team in developing a comprehensive care plan to overcome identified needs/barriers.
- Provides comprehensive care coordination for Clinica patients to include identifying and overcoming medical, psych-social, community, cultural/religious and financial barriers to care.
- Coordinates with internal and external resources to meet the identified needs of the patients and collaborates with providers.
- Functions as communication expert on social services, when working with other departments.
- Manages and monitors mental health referrals to Community Mental Health Centers for patients with higher level of behavioral health needs.
- Responsible for follow-up of high-risk patients, especially patients with depression as indicated by the depression registry’s targeted outcome measures.
- Responsible for resolving conflicts in a professional manner and is a model for strong interpersonal communication skills with all staff.
POSITION QUALIFICATIONS:
Education and Experience:
- BA/BS degree in Health/Human Services or a related field OR at least five years of equivalent experience in coordinating care for patients with the highest level of behavioral health and substance use treatment needs.
- Certified Addiction Specialist (CAS) preferred.
- Minimum 2 years of experience providing health education and/or patient navigation required, with preference for behavioral health navigation.
- Experience working in a community health setting or with underserved populations preferred.
Knowledge, Skills and Abilities:
- Bilingual in Spanish required.
- Sensitivity to under resourced individuals and communities.
- Case management for complex patients.
