Job Title: Street Medicine Healthcare Provider – Nurse Practitioner

Job Summary:
The Street Medicine Healthcare Provider, Nurse Practitioner (NP), is a critical member of the Street Medicine Program. This role is dedicated to delivering compassionate, high-quality medical care to individuals experiencing homelessness. The provider will address the complex health needs of unhoused individuals, including chronic disease management, acute care, and preventive services. They will work in various environments, such as encampments, shelters, and transitional or permanent housing, to ensure access to healthcare for those most in need.
Key Responsibilities:
- Conduct comprehensive medical evaluations, develop treatment plans, and manage chronic and acute conditions, including diabetes, hypertension, and infectious diseases.
- Administer preventive care services such as immunizations, screenings, and health education tailored to the unique needs of the unhoused population.
- Provide acute care for common conditions like respiratory infections, skin disorders, and injuries, emphasizing harm reduction and continuity of care.
- Travel with the mobile unit to encampments, shelters, and community sites to bring care directly to patients.
- Collaborate with a multidisciplinary team, including social workers and community health workers, to address social determinants of health.
- Maintain detailed and accurate electronic health records to ensure quality care and compliance.
- Participate in community health initiatives, advocacy efforts, and outreach activities to foster trust and build lasting relationships within the community.
Qualifications:
- Certified Nurse Practitioner with active state licensure.
- At least two years of experience in community health, urgent care, or family medicine, preferably with vulnerable populations.
- Deep understanding of the unique health challenges faced by unhoused individuals.
- Strong interpersonal skills, cultural humility, and a passion for serving marginalized populations.
- Bilingual skills in Spanish preferred.
Physical Requirements:
- Ability to stand, walk, and move for extended periods while providing care in various environments, including encampments, shelters, and mobile units.
- Capacity to lift, carry, and transport equipment or supplies weighing up to 30 pounds.
- Comfort working outdoors in varying weather conditions and navigating uneven terrain.
- Ability to bend, stoop, and kneel as needed during patient care or outreach activities.
- Adequate manual dexterity to perform medical procedures, operate equipment, and manage electronic health records on portable devices.
- Visual and auditory acuity to perform essential functions such as patient assessments and effective communication.
- Capability to drive and sit for extended periods while traveling to outreach locations.
Salary : $150,000 - $165,000
