The ideal candidate will work in a critical access hospital setting, providing care coordination services to patients through cross collaboration with multidisciplinary teams. They will evaluate options and develop patient discharge plans that meet individual needs.
This role is part of the Care Coordination team, working alongside specialized case managers holding their CCM Commission for Case Manager Certification. The successful candidate will have previous experience working as a Case Management Registered Nurse in an inpatient population within an acute care setting.
Key Responsibilities:
- Collaborate with healthcare teams to develop effective patient care plans
- Evaluate patient needs and develop discharge plans that meet individual requirements
- Provide education and support to patients and families
- Coordinate with external agencies and providers to ensure seamless care transitions
Requirements:
- Active registered nurse license
- Previous experience in case management or a related field
- Knowledge of best practices, state, federal, and hospital regulatory compliances and guidelines
- Familiarity with discharge planning and utilization management
