Job Description

The Case Management Registered Nurse will work in a critical access hospital setting and be responsible for providing care coordination services to patients through cross-functional collaboration with multidisciplinary teams.

Required Skills and Qualifications

  • Active registered nurse license
  • Previous Case Management experience working with an inpatient population within an acute care setting
  • Up-to-date knowledge of best practices, state, federal, and hospital regulatory compliances and guidelines
  • Prior discharge planning assessment and utilization management experience
