In this full-time role, you will educate staff, physicians, patients, and families about the case management process, ensuring that new admissions and continued stays meet MCG criteria. You will engage with physicians to establish the most accurate DRG, length of stay, and expected discharge dates while interfacing with third-party payors to assist with certification and discharge planning.
The RN Care Manager will review patient records and collaborate with the interdisciplinary team to gather comprehensive information for effective communication with payors, aiming to prevent denials. This role also involves participating in interdisciplinary rounding to identify areas for improvement and facilitating communication between patients, families, and social work to ensure safe and appropriate discharge plans.
We are looking for a candidate with over three years of diverse RN experience, preferably with a background in critical care, home care, or case management. A Bachelor's degree in Nursing (BSN) and case management certification are also preferred.
