Job Description
We are seeking an experienced Registered Nurse (RN) to serve as our Care Manager. This pivotal position is responsible for ensuring seamless case management processes and optimal patient outcomes.
Responsibilities

- Collaborate with physicians to establish accurate DRG, length of stay, and expected discharge dates, while interfacing with third-party payors to facilitate certification and discharge planning.
- Review patient records and work with the interdisciplinary team to gather comprehensive information for effective communication with payors, aiming to prevent denials.
Requirements
To be successful in this role, you will need:
- Over three years of diverse RN experience, with a background in critical care, home care, or case management preferred.
- A Bachelor's degree in Nursing (BSN) and case management certification are also preferred.
