The Case Management Registered Nurse will work in a critical access hospital setting and provide care coordination services to patients through cross collaborating with multidisciplinary teams, evaluating options and developing patient discharge options that meet an individual's need. The Case Management Registered Nurse will join the Care Coordination team, working alongside a team of specialized case managers holding their CCM Commission for Case Manager Certification. Ideally the Case Management Registered Nurse will have previous Case Management experience working with an inpatient population within an acute care setting. Additionally, this individual will be up to date on best practices, state, federal and hospital regulatory compliances and guidelines, have prior discharge planning assessment and utilization management experience, and hold an active registered nurse license.
