This health center is a dynamic and community-focused healthcare provider serving their town for over 100 years. The Certified Diabetes Educator will collaborate with Registered Nurses and Board Certified Providers on care plans for patients diagnosed with Diabetes. The Diabetes Educator will lead a variety of group classes as well as provide one-on-one coaching.
