A 35 bed Medical facility outside of Prairie City, Oregon is looking to bring on a Clinical Social Worker to join the team. This hospital has 100 wonderful staff members including a handful of staff SW's.
This Hospital is looking for a Clinical Social Worker to join their outstanding team. This Social Services department serves patients who need behavioral health assessments, care management, and interventions in the primary care setting. This department works closely with almost all areas but predominantly Behavioral Health. The Social Services department is looking for a LCSW Social Worker who can:
- Validate patient rating scales and track patient progress
- Provide wraparound treatment ranging from PCP to the Psychiatric Consultant.
- Revise treatment plans when patients are not progressing.
- Provide Brief Psychosocial interventions as needed.
- Maintain a registry of patient progress.
