Responsibilities of the Hospital Social Worker LCSW will include assessing a patient's psychosocial needs, developing individualized treatment plans, and providing external resources or referrals as needed. The Hospital Social Worker LCSW will recommend appropriate levels of care, and will assist with coordinating and securing inpatient psychiatric treatment and insurance authorizations as needed. The Hospital Social Worker LCSW is also accountable for completing all written reports in a timely fashion, maintaining patient records as required by law and Medical center policy, and continuously cross-collaborate with multidisciplinary teams, case management, specialists and external providers as needed.
The Hospital Social Worker LCSW should have previous clinical social worker or mental health consultant experience
