The Ambulatory Care Coordination team provides comprehensive care coordination services to pediatric patients and their families across various specialty clinics and outpatient departments. Job Responsibilities:
- Assess and evaluate the needs of pediatric patients and their families to develop individualized care plans
- Coordinate and facilitate appointments, referrals, and follow-up care for patients within the ambulatory setting
- Collaborate with healthcare providers to ensure seamless transitions of care and continuity of services
- Provide education and support to patients and families to promote self-management and adherence to treatment plans
- Utilize evidence-based practice guidelines and standards to optimize patient outcomes and resource utilization
- Participate in interdisciplinary care team meetings and case conferences to discuss patient progress and care coordination strategies
