The Nurse Case Manager will play a critical role in coordinating and managing a client's care across various sectors, including health care, social services, and insurance. The Nurse Case and Utilization Manager coordinates the necessary services, such as medical care, housing, and financial assistance, ensuring that the care plan is effectively implemented. This person will monitor and evaluate the clients' progress, making adjustments to the care plan as needed to improve outcomes. Additionally, as the Case Manager, you will advocate for clients, ensuring they receive appropriate and effective services, and provide emotional support and guidance to both clients and their families.
The Nurse Case Manager will directly report to the Chief Nursing Officer and will work with an established team of RN’s, physicians, occupational therapists, physical therapists, and supporting team to ensure the patient is receiving the highest quality care.
