Job Description
Clinical Management Consultants is seeking a highly skilled Nurse Case Manager to play a vital role in enhancing patient-centered care. As a key member of our team, you will be responsible for coordinating and managing clients' care across various sectors, including healthcare, social services, and insurance.

As a Nurse Case Manager, you will advocate for clients, ensuring they receive appropriate and effective services. You will also provide emotional support and guidance to both clients and their families.
You will directly report to the Chief Nursing Officer and work with an established team of RNs, physicians, occupational therapists, physical therapists, and supporting staff to ensure patients receive the highest quality care.
