CSO is looking for Transitional Counselors to work in our Emergency Services Program (ESP.)
Our Emergency Service Programs (ESPs) offer clients behavioral health assessments, intervention and stabilization (for youth, families and adults,) 24-hour availability, and are a preferred alternative to hospital emergency departments. We provide daily assessment and brief counseling, case management, nursing support and peer support.
This position provides short-term case coordination, outreach and support to CBHC clients, individuals utilizing Respite beds, or being screened for eligibility for Department of Mental Health (DMH) services. The Transitional Counselor serves as the primary coordinating resource linking individuals not already known to the DMH system while service eligibility is being reviewed.
As a Transitional Counselor:
You Will:
- Provide assessment of independent living skills and skills training in areas including transportation, money management, cleaning, laundry, food preparation, personal grooming, and health care
- Document a transition plan based on assessed needs and listing transition goals.
- Assist with advocacy needs; facilitate "mediation" as appropriate
- Assist in identifying and obtaining entitlements including housing and tenancy engagement
- Provide intensive support to consumers during psychiatric crises or periods of transition.
- Assist consumer with identifying appropriate insurance and benefit application and accessing services of choice
- Assist with developing and maintaining a "natural support network" of family and friends
- Assist consumer in negotiating relationships with roommates, landlords, and others
- Coordinate development of meaningful day activities, including clubhouse participation, schooling, employment, or volunteer activities
- Assist with finding or developing recreational and social activities of the consumer's choice.
- Provide transportation for consumers as appropriate, following program guidelines
- Maintain appropriate contacts with family members, others in the "natural helping network," DMH case managers, outpatient therapists, and others as necessary to support consumers
- Participate in team discussions regarding program consumers in order to provide back up for other team members as needed
- Participate in design, review, and revision of Program Specific Treatment Plans with consumer, family members, and other providers
- Participate in team meetings, ISP meetings, inter-program and inter-agency case conferences, and other interactions with other providers to promote integrated consumer services
- As a designated critical employee of CSO, must come to work when scheduled regardless of adverse weather conditions, and when other CSO offices are closed, including holidays. In the event there are extenuating circumstances, employee must make prior arrangements with Program Director in order to maintain adequate staffing in the program
- Complete all initial and ongoing credentialing requirements for those hospitals where employee will be providing services including obtaining required vaccinations, tox screens, and safety/health education competencies

Qualifications
Your Background:
- Bachelor's or Associate’s degree in a mental health discipline (or High School diploma and five years of related experience)
- Two years of experience in community mental health setting (preferred)
- Computer proficiency
- Valid driver's license and vehicle
