What are the responsibilities and job description for the Safe Haven Case Manager position at Clinical & Support Options Inc?
CSO is looking for a Case Manager for our Safe Haven Program.
Our Safe Haven is a 24 hour/day, 7 day/week fully supported transitional housing program. Safe Haven provides beds for chronically homeless individuals residing in Pittsfield, MA (Berkshire County). This program is peer supported and staffed with Counselors and Case Managers.
This position will provide intensive, clinical & housing case management to consumers with serious mental illness needing such support, or who have been hard to engage in the mental health system. The Case Manager will have regular contact with consumers, and will provide support services for family members and others in the "natural helping network."
As a Safe Haven Case Manager:
You Will:
- Provide skills training and assistance in areas of daily living activities including transportation, money management, cleaning, laundry, food preparation, personal grooming, and health care
- Assist with advocacy needs; facilitate "mediation" as appropriate
- Assist in identifying and obtaining entitlements, which meet consumer's needs.
- Assists clients in the development of individual plans to move toward self-sufficiency and to retain/secure permanent housing. Evaluates, facilitates and monitors clients progress towards established goals and objectives. Documents in individual case file/ETO
- Provide resource/assistance for clients in identifying and removing barriers to housing success, as well as other competencies.
- Advocate for clients with other service providers, including federal, state and city agencies and other non-profits; provide referrals to other appropriate service providers as needed. (e.g. Social Security, Medicaid, Medicare)
- Provide intensive support to consumers during psychiatric crises or periods of transition
- Assist consumer with identifying appropriate mental health and medical services and accessing services of choice
- Assist with developing and maintaining a "natural support network" of family and friends
- Assist consumer in maintaining the appropriate upkeep of apartment or shelter space that will ensure healthful and safe living
- Assist consumer in negotiating relationships with others
- Coordinate development of meaningful day activities, including employment or volunteer activities
- Assist with finding or developing recreational and social activities of the consumer's choice.
- Provide transportation for consumers as appropriate, following program guidelines
- Maintain appropriate contacts with family members, others in the "natural helping network, outpatient therapists, and others as necessary to support consumers
- Participate in design, review, and revision of treatment plans with consumer, family members, and other providers

Your Background:
- Bachelors degree in mental health field, strongly preferred
- Two years of experience in community mental health setting (preferred)
- Computer Proficiency
- Valid drivers license and reliable transportation
