MDS COORDINATOR
JOB DESCRIPTION
The MDS Coordinator has a vital role in the overall direction for resident care and rehabilitation. The data collection and plans made for each resident can determine their decline, maintenance and/or their rehabilitative potential. To this end, it is important that the person who fills this role be able to facilitate communication between Nursing, social services, activities, dietary and rehab departments, understanding and respecting what each professional brings to resident care. And finally, be able to clearly and concisely communicate the plan of care to the resident, Doctor, family and other nursing home staff.
QUALIFICATIONS
o Licensed in the state of Arkansas as an Licensed Practical Nurse (LPN) or Registered Nurse (RN).
PROFESSIONAL REQUIREMENTS:
o Skilled at organizing various duties.
o Must be flexible and able to handle change easily and professionally.
o Must display a willingness to continue to study, learn and implement continuously changing Medicare regulations.
o Maintain a positive attitude.
o Communicate with residents, families and staff in a professional and tactful manner.
o Be a role model of professionalism and teamwork.
o To assure that the MDS and completed care plan is well within the regulatory guidelines of 31 days or less.
o To gather data and complete Resident Assessment Instrument (RAI) in an accurate and timely manner.
o To coordinate efforts from all resources probable, such as residents, staff, health care professionals and the care plan team to achieve optimal plan for each individual plan of care.
o To assure the accurate completion of the documentation regarding summaries and updates for care plan.
o To assure that the completed plan of care is clear, concise, and pertinent to each individual resident.
PHYSICAL REQUIREMENTS:
Walking, reaching, bending, lifting, grasping, pushing and pulling, fine hand coordination, ability to hear and respond to pages, ability to understand and follow written and oral instructions and directions; ability to communicate with residents, families, personnel, vendors and consultants; ability to distinguish smells, tastes, and temperatures; and ability to remain calm under stress.
RESPONSIBILITIES
o Collecting resident care data from various sources.
o Coordinating the Assessment Reference Date with the Director of Rehab to best maximize resident care levels.
o Preparing the MDS for electronic transfer to the State on a weekly basis.
o Completion and implementation of the plan of care.
o Accurately documenting resident direction and achievement according to the plan of care.
o Assisting the resident in maintaining or improving their health status in the facility.
o Coordinating resident care with the Director of Nursing and relaying any issues or concerns regarding completion of the MDS/Care Plan/and concerns regarding direct patient care.
o Leading the MDS/Plan of Care meeting and assuring timely and accurate completion of all portions of the MDS.
o Preparing monthly schedules for MDS/Plan of Care meeting for all disciplines and distributed on the first of every month.
o Assisting with letters to family members and or Resident responsible party inviting them to the Care plan meeting.
o Assessment and Care Planning will be completed to meet the needs of the newly admitted resident, prior to the completion of the first comprehensive assessment.
o Interdisciplinary expertise will be used to develop a plan of care to improve a resident's functional abilities.
o The care plan will be oriented toward preventing avoidable decline in functioning or functional levels.
o Treatment objectives will have measurable outcomes.
o The care plan will attempt to manage risk factors.
o The care plan will build on the resident's strengths.
o The care plan will reflect standards of current professional practice.
o The resident will be given sufficient information about his or her treatment so that an informed choice can be made.
o The resident's wishes and goals will be obtained.
o If the resident refuses treatment, the documentation and care plan will reflect the facilities efforts to find alternative means to address the problem.
o Families and other resident representatives will be involved in care planning.
o Direct care staff will be fully informed about the care, services, and expected outcomes of the care they provide.
