Job Details:

Clover Home Health has contracted with a major insurance provider in Indiana to complete In-Home Health Assessments with their new members. Clover Home Health is seeking to hire a Registered Nurse (RN) Case Manager (minimum 2-3 days per week availability) who will evaluate the members in their home and complete a full health, social, and behavioral needs assessment with each member. These are ONE-TIME only visits--all assessments, documentation, notations, and referrals will be submitted to the insurance providers' in-house case management team. The nurse must be centrally located within the assigned counties stated in this job overview.

These are paid per visit of $150 per RN visit plus mileage. The expectation is to complete a minimum of 3-5 visits per week with the ability to complete up to 10 visits weekly. Each visit will take an average of 1.5 hours based on member/members scheduled for a full health assessment. Ideal Registered Nurse candidate must live in the South-West Indiana region. 75% off all visits are with the Vanderburgh county area.

Our ideal Registered Nurse candidate must have strong health assessment skills, be able to recognize critical health needs, and act quickly to support client care.

Responsibilities:

• Evaluate and complete head-to-toe health, behavioral, and social determinate assessments in the home with each assigned member. Help the members login to their Member Portal and educate them on how to navigate and find important health information.
• Use clinical tools, data, and other information to complete an assessment of client needs.
• Document complex medical information to outline and improve health results that help determine immediate and routine care for the client.
• Educate clients and families on benefits, services, and other resources available to them.
• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
• Assist members with assignment of a PCP and scheduling appointments.
• Demonstrate knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapies.

Requirements:

• Current nursing license required for the state of Indiana.
• Bachelors (preferred) or Associates Degree in Nursing from an accredited nursing school.
• Minimum of one years Home Health or Case Management Nurse experience.
• General knowledge of case management practices, including familiarity with relevant state and local services.
• Current CPR/First Aid certification In-Person, Current TB screening (past 12 months).
• Valid Driver's License and Automobile Insurance, dependable transportation.
• Excellent communication and listening skills.
• Tech savvy, able to learn and navigate two portal systems. Use of Apple Products.
• Perform routine duties independent of supervision but consults with others as required.

Job Type: Part-time

Pay: $450.00 - $1,500.00 per week
