Position Description:
We are looking for a dedicated Case Manager for our Skilled Nursing Facility ( SNF ) in Lawrence, New York. This is a full-time, in-person position, Monday - Friday 9am - 5pm.
Qualifications of the Case Manager:
Valid Licensed Practical Nurse ( LPN ), Registered Nurse ( RN ), and Licensed Social Worker ( LSW) licenses are required by state regulations, with a background in geriatric or long-term care.
Familiarity with care transitions, discharge planning, and interdisciplinary coordination in a Skilled Nursing Facility ( SNF ) or similar healthcare setting.
Understanding of Medicare, Medicaid, managed Care, and private insurance guidelines for admissions, authorizations, and continued stay coverage.
Proven ability to work with residents, families, hospitals, physicians, and internal teams to ensure seamless care planning and placement.
Proficient in EMRs and able to document case notes, progress, and coordination accurately and timely.
Responsibilities of the Case Manager:
Work with hospitals, families, and referral sources to coordinate smooth transitions into and out of the facility.
Collaborate with nursing, therapy, dietary, and social services teams to create and update personalized care plans based on residents’ needs.
Serve as the central point of contact for families, residents, and staff to ensure consistent updates and alignment on care goals.
Ensure proper authorization is in place for skilled services, submit updates to insurance companies, and help prevent unnecessary denials.
Help residents and families navigate healthcare decisions, community resources, and end-of-life care, when applicable.
