Position Description:
We are looking for an experienced Licensed Medical Social Worker ( LMSW ) for our Skilled Nursing Facility ( SNF ) in the Staten Island, New York area to assist residents who are struggling with mental health issues. This is a full time, in person position.
Qualifications:
Valid Licensed Medical Social Worker ( LMSW ) license in New York
2 years of experience as a Licensed Medical Social Worker ( LMSW )
2 years of experience working in a post acute medical setting
Responsibilities:
Assess patients through sessions and medical history reviews
Develop comprehensive treatment plans
Maintain complete and confidential medical records
