Position Description: We are looking for a skilled and experienced MDS Coordinator to join our Skilled Nursing Facility ( SNF ) in Yonkers, New York. This is a full-time, in-person position Monday - Friday 9am - 5pm. Qualifications of the MDS Coordinator : 2 years of experience as an MDS Coordinator in a post acute setting Bachelor's degree in nursing or a related field (Master's degree preferred) Valid Registered Nurse (RN) License in New York Familiarity with HEDIS measures and quality improvement initiatives Responsibilities of the MDS Coordinator : Perform evaluations of patients using the MDS assessment tool, covering Review MDS data to identify trends, monitor patient health changes, and Conducting initial assessments on new residents to establish baseline Participating in discharge planning process to ensure patients receive Monitor staff to ensure quality care is provided and in line with Medicare/Medicaid regulations If you're a diligent and detail oriented MDS Coordinator looking for a challenging and rewarding opportunity, we encourage YOU to apply! #MISCEZ
Compensation: $115,000- $125,000 Annually
