Position Description:
We are looking for a dedicated MDS Coordinator for our Skilled Nursing Facility ( SNF ) in Centerville, Ohio. This is a full-time, in-person position, Monday - Friday 9am - 5pm.
Qualifications of the MDS Coordinator:
Valid Registered Nurse ( RN ) or Licensed Practical Nurse ( LPN ) license in the state of Ohio.
2 years of experience in an acute or Long Term Care ( LTC ) setting, previous experience in MDS Coordination or a related role is preferred.
Certification as a Resident Assessment Coordinator ( RAC-CT ) through a recognized organization, such as the American Association of Nurse Assessment Coordination ( AANAC ), is often required or preferred.
Proficiency in MDS 3.0 assessments, a solid understanding of data collection, reporting, and analysis, and familiarity with medical-legal frameworks are essential. Strong organizational, communication, and interpersonal skills are also important.
Responsibilities of the MDS Coordinator:
Administer and manage assessments of residents using the Minimum Data Set ( MDS ) tool to evaluate their health status and care needs.
Develop individualized care plans based on assessment data, ensuring that they address the medical, nursing, mental, and psychosocial needs of each resident.
Ensure that all assessments and care plans comply with federal and state regulations. Maintain accurate and up-to-date documentation for each resident.
Collaborate with other healthcare professionals, including physicians, nurses, therapists, and social workers, to ensure comprehensive care for residents.
