Position Description:
We are looking for a dedicated Wound Care Nurse Practitioner ( NP ) or Physician assistant ( PA ) for our Skilled Nursing Facility ( SNF ) and Home Health Services in the state of Oregon. This is a full-time, in-person position, Monday - Friday 9am - 5pm.
Qualifications of the Wound Care Nurse Practitioner ( NP ) or Physician Assistant ( PA ):
- Valid Nurse Practitioner ( NP ) or Physician Assistant ( PA ) license in Oregon.
- Wound Care Certification ( WCC ), Certified Wound Specialist ( CWS ) is preferred.
- 1 years of experience in managing wound care and infection prevention in a Skilled Nursing facility ( SNF ) or Home Health setting.
- Conduct wound assessments and provide personalized treatment plans for patients.
- Complete documentation accurately and in a timely manner.
- Educate patients and their families on wound care prevention.
- Monitor wound healing progress and make adjustments to treatment plans as needed.
