Compensation: $130,000 - $145,000 Annual Salary, based on experience

Position Description:
We are looking for a dedicated Nurse Practitioner ( NP ) for our Skilled Nursing Facility ( SNF ) in Phillipsburg, New Jersey for bedside patient care. This is a full-time, in-person position, Monday - Friday 9am - 5pm.
Qualifications of the Nurse Practitioner ( NP ):
Valid Nurse Practitioner ( NP ) license in the state of New Jersey.
1 years Long Term Care experience.
Responsibilities of the Nurse Practitioner ( NP ):
Upon admission to the facility, examine residents and their medical records.
Checks ups, management of chronic conditions and medications, transitional care management, discharge planning and family engagement.
