Position Description: We are looking for a dedicated and compassionate Regional MDS for our Skilled Nursing Facility ( SNF ) in Lawrence, NY, who is responsible for overseeing medical operations and ensuring the delivery of high-quality healthcare services across a designated region. The Regional MDS role involves leadership, strategic planning, and collaboration with healthcare teams to improve patient o0utcomes and operational efficiency. This is a full time position, in person position Monday - Friday 9am - 5pm Qualifications: ? Valid Registered Nurse ( RN ) and Licensed Practical Nurse ( LPN ) license in the state of New York ? 2 years of experience as a Regional MDS ? Experience working in a Skilled Nursing Facility ( SNF ) Responsibilities: Perform evaluations of patients using the MDS assessment tool, covering areas like physical function, cognitive abilities, medical conditions, and psychosocial needs. Utilize data from MDS assessments to create individualized care plans with specific goals and interventions for each patient. Review MDS data to identify trends, monitor patient health changes, and ensure accurate documentation for billing purposes. Conducting initial assessments on new residents to establish baseline data and develop appropriate care plans.
