Clinical Forms Specialist / Supply Coordinator Benefits :

The Clinical Forms Specialist / Supply Coordinator is responsible for filling out all necessary patient medical forms, accurately and in a timely manner, while adhering to strict guidelines and regulations to maintain patient confidentiality. The position is responsible for ordering and maintaining medical and janitorial items as well.

Clinical Forms Specialist / Supply Coordinator Responsibilities :

  • Accurately input patient medical history, diagnoses, medications, treatment plans, and other clinical data requested into the EMR system.
  • Review and complete all required clinical forms (i.e. : FMLA, PFL, disability), ensuring accuracy and completeness.
  • Regularly review completed forms for consistency, clarity, and adherence to medical documentation standards, identify and correct any errors or missing information.
  • Maintain strict confidentiality of patient medical records by following HIPAA guidelines and protecting sensitive information.
  • Communicate with physicians, nurses, and other healthcare providers to clarify clinical information and ensure accurate documentation.
  • Stay updated on all relevant healthcare regulations and policies regarding medical documentation and reporting.
  • Prepares requisitions for supplies and equipment to keep adequate stock on hand; Verifying / expediting with suppliers, evaluating substitutions, expiration dating, accounting, etc.
  • Order special items as requested; orders certain standard items directly from an approved vendor.
  • Receives and inspects supplies and equipment.
  • Checks items against invoices or freight bills for proper quantity.
  • Checks items for damage and notifies the vendor or transportation company of unacceptable items; marks discrepancies on invoices; accepts delivery on items and records items received.
  • Stores items received on shelves, in bins or in other storage areas.
  • Perform other related duties as assigned.
  • Clinical Forms Specialist / Supply Coordinator Qualifications :

  • Associates Degree and two years relevant patient services experience or equivalent combination of education and experience required in a health care setting.
  • Knowledge of medical terminology, medical authorization, familiarity with medical coding and excellent written / oral communication skills required.
  • Two years experience using Electronic Medical Records (EMR). Experience using Medent preferred.
  • Two years experience completing medical leave / disability related documentation.
  • Familiarity and experience with New York State Immunization Information System(NYSIIS).

We are a Family Oriented Medical practice with Physicians, Physician Assistants, Nurse Practitioners and Diabetic Educators. CNY Family Care takes a team approach to meeting the medical needs of our patients through compassion, respect, and professional care. We are available for our patients when they need us. We partner with our patients to provide a proactive approach to medical care, which improves their quality of life.

    One of our main goals at CNY Family Care, LLP is to provide all of our patients with continuous and comprehensive health care. As part of this care model, we ask all of our patients to select one personal physician. This PCP / Primary Care Physician will maintain an ongoing relationship with the patient and will then always be in charge of their care. They will oversee the care team that consists of nurses, Nurse Practitioners, Physician Assistants and Diabetic Educators who might interact with the patients when they visit the office. Our care team will work towards ensuring that the patients receive the appropriate age-related preventative testing and treatment as well as the latest evidence-based treatment (proven and documented) for their individual medical condition(s).

    We continue to grow and expand our services in order to meet the demands of our patient population.
