What to expect:

• Administer or assist with medication for residents according to the Resident Care Plan.

• Engage residents while checking on their wellbeing and communicate changes in condition or Care Plan.Assists with taking resident intakes for admission to facility.

• Consulting with team members on care management for patients with chronic conditions

• Documenting patients’ medical progress

Successful Candidates:

• LPN license in good standing with all federal and state agencies.

• Demonstrated leadership abilities.

• Proven ability to communicate effectively to staff, residents and family members.

Rate Range: $39 - $41/hour
