Overview
We are seeking a dedicated and compassionate Mental Health Case Manager to join our team. In this role, you will be responsible for providing comprehensive support and guidance to individuals facing mental health challenges. Your expertise in crisis management and behavioral health will be essential in helping clients navigate their mental health journeys.
Responsibilities
- Conduct thorough assessments of clients' mental health needs and circumstances.
- Develop and implement individualized care plans that address clients' specific goals.
- Provide crisis intervention and management to clients in urgent situations.
- Facilitate access to community resources, including housing assistance (Section 8) and childcare services.
- Offer support in addiction counseling and recovery processes.
- Utilize motivational interviewing techniques to empower clients toward positive change.
- Collaborate with multidisciplinary teams, including social workers and healthcare providers, to ensure comprehensive care.
- Maintain accurate documentation of client progress and interactions in compliance with regulatory standards.
- Advocate for clients' rights and needs within the healthcare system.
Skills
