Under the direction of the Program Manager, the Time Limited Subsidy Case Manager formerly known as Rapid Rehousing Case Manager will provide services to homeless transition aged youth (18-24 years old). The Rapid Rehousing Case Manager will be responsible for assessing needs and connecting youth to housing that is appropriate for their needs. This may include (but not be limited to) crisis / bridge, housing, permanent supportive housing, shared, or general affordable housing. The Rapid Rehousing Case Manager will assist youth in managing their own lives and achieving goals as established in their Individual Action Plan. Provide each youth on case load with referrals for services that includes Mental / Physical Health Provisions, mainstream benefits, education, employment, and vocational training.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Conduct initial intake, needs assessment, and collect the Next Step screening tool to enroll client to the Time Limited Subsidy Program
- Carry case load of maximum of 20 -25 youth.
- Assist clients in determining personal goals and plans to achieve such goals through the development of the Individual Action Plan (IAP), including frequency and duration of any identified support services.
- Conduct face-to-face contact with each client on regular basis : once a week for unhoused clients and a minimum of one home visit once housing is secured.
- Partner with partners to outreach to homeless youth in South Los Angeles.
- Establish and cultivate relationships with landlords and other housing providers to build a portfolio of housing options for clients.
- Coordinate services in the following categories : (a) employment training and education; (b) money management counseling; (c) mental and / or medical health services and (d)legal and benefits counseling.
- Participate in coordinated case conferencing meetings with service providers.
- Develop and facilitate workshops on topics including, money management, life skills (including transportation, training in self-advocacy / community accessibility), achieving independence and other topics as needed.
- Data collection / entry into HMIS & Salesforce database to assemble reports and evaluate program outcomes.
- Maintain client case file for accuracy of all eligibility required documents as per CRCD case filing protocols and procedures.
- Compile and submit monthly service reports to supervisor that track client’s status and the supportive services which have been provided to them.
- Attend all required LAHSA meetings, relevant community meetings and staff development training as assigned.
- Act as a liaison among the Project Manager, the property management company, and the primary mental health provider to facilitate the communication of concerns that could jeopardize the client’s housing.
- Act as a liaison and coordinate linkage to services with numerous public social service agencies and community provider to link clients with employment and education / training.

SALARY RANGE
45,000-$55,000 annually
