PURPOSE OF POSITION
This position requires thorough knowledge and understanding of human rights, strengths-based approach in serving clients as well as social work skills (particularly in crisis intervention and conflict resolution) and cultural competency. The Shelter Assistant Case Manager should be familiar with and employ key social work values, including client empowerment, professional boundaries, advocacy, and self- care. They should also possess client management expertise, independent decision-making ability, and excellent interpersonal and team-building skills. This position requires fieldwork and travel throughout Los Angeles County to work with community partners to provide advocacy and services to trafficking survivors in the Los Angeles region. Full time, hourly position.
Assistant Shelter Case Management: Assist Shelter Case Manager in providing survivor-driven, client-centered timely and professional case management for domestic and foreign national victims of human trafficking.
· Assist Case Manager, as needed, in conducting safety assessments to determine eligibility to enter shelter;
· Assist Case Manager, as needed, in providing linkage to appropriate resources, follow up, and tracking outcomes;
· Under the direction of the Shelter Case Manager, help clients coordinate, health, mental health, legal, employment, education, criminal/civil case, and DPSS appointments and accompany/drive clients as needed;
· Under the direction of the Shelter Case Manager, assess the need for and distribute emergency funds and resources to clients;
· Ensure that residents' rights and needs are protected;
· Provide or coordinate client accompaniment as needed;
· Manage case files and provide detailed progress notes in hard copy files and in database on a daily basis;
· Assist in facilitating house meetings and workshops for residents;
· Able to work independently and respond to crisis and/or emergency situations; and
· Uphold policies and procedures that contribute to the safety and confidentiality of the CAST Program.
Client Advocacy and Outreach: Ensure that client rights and needs are protected at local service delivery level and by service providers.
· Identify barriers to services and advocate for higher quality of services with external service organizations;
· Ensure protection of identify and confidentiality in line with survivor's safety, needs and preferences; and
· Advocate for culturally and linguistically appropriate services for clients.
Intake Process: Manage hotline calls, intakes, and orientation during intake hours
· During intake hours, answer hotline calls from CAST's 24/7 hotline, and respond accordingly;
· Perform client intake, screen new referrals, triage needs, create initial action plans, and coordinate with client, CAST staff, and partners to address emergency needs.
· Inform on-call Emergency Response staff of any urgent calls that required immediate follow-up from the hotline;
· If eligible conduct shelter intake and admit into Hummingbird Haven Shelter Program
· Document all hotline calls in the database
· Maintain and obtain community resources
Database Management: Manage database information as it pertains to shelter intakes and shelter client records.
· Ensure shelter intakes are completed correctly and with all information
· Update and ensure client statuses (enrollment date and/or exit date)
· Upon entry to program convert hotline record to client record, assign Case Manager, and assign grant
· Schedule monthly check in's with staff to review hotline records and shelter intakes to ensure quality control
· Ensure hotline records and shelter intakes are being completed accurately
· Enter required progress notes and other information into our database completely, accurately, and on a timely basis
· Provide ongoing training with staff to update on any changes, trends or provide additional database training needed.
· Input data into the city's database system to track clients served and progress on grant contract goals.
Client Orientation and Exit Process: Ensure the orientation and exit process is being executed properly by all Shelter Staff
· Familiarizing new clients to our site and our program, ensuring that they understand the requirements of the facility
· Train and supervise staff procedure on preparing beds, inspecting new client belongings upon entrance of program, and distributing any basic needs new participants may need
· Train and supervise that each new participant is completing the 48 hour assessment
· Upon exit assist in scheduling shelter staff in ensuring the bed and other facilities used by the participant is clean and ready for next participant.
· Ensure exit paperwork is completed and documented in database
REQUIREMENTS
Individual must be organized, flexible, capable of multi-tasking, and able to identify and respond to shifting priorities. Must also be a self-starter that can work within a team environment and handle a variety of tasks with urgent and fluctuating deadlines while maintaining strict attention to detail and balancing short- and long-term goals. Specific requirements:
1. B.S.W. or BA in social work with at least 2 years' experience in housing (i.e. shelter services, Rapid Rehousing, Affordable Housing, vouchers, etc.), providing case management services and/or direct services with highly traumatized populations; M.S.W. or MA preferred.
2. Bilingual in Spanish or one or more Asian Pacific Islander (API) languages preferred
3. Ability to work independently and with minimal supervision; Must be organized, flexible, detail-oriented, and multi-tasking.
4. Must be a self-starter, able to work in a team environment and manage time effectively.
5. Strong assessment skills and experience to determine client needs;
6. In-depth knowledge of issues related to providing services to survivors of human trafficking, domestic violence, or sexual violence preferred, including knowledge of the service delivery system in Los Angeles and the regulations governing public and private benefits as well as federal benefits available to trafficking survivors.
7. Demonstrated sensitivity to and knowledge of issues involved in working with diverse populations and/or with individuals who have limited English proficiency.
8. Knowledge of multidisciplinary and multicultural approaches;
9. Ability to maintain a positive, cooperative, and professional demeanor with clients, partner organizations, volunteers/interns, staff/board, donors, consultants, and members of the public.
10. Excellent oral and written communication skills, and ability to deliver oral presentations to diverse professionals and audiences;
11. Ability to operate computer systems, including Internet, email, databases, and word processing software (Microsoft Office Suite);
12. Capacity to work in a fast-paced program and manage multi-faceted services;
13. Available to travel extensively throughout the Los Angeles region - must have a car, insurance and a valid driver's license
